A system generated ID and Password was used to enter your application information into our system. (Privacy Policy)
This ID and Password are not known by PHA staff.
If you supplied an E-Mail address on your application information, you may use that E-mail address to obtain the ID and
Password associated to your account.  Click here to go to the ID and Password request page.
Otherwise, please send a request to Support@My-PHA.org to assist you with obtaining your ID and Password.

Please note that you will not be allowed to create a new account using information already in our system.

Registration for New Members.

Registration

This is an example of  a filled in Registration Page:

The sections below describe what each field is, and what kind of data is acceptable.
When you register, our system looks to see if you already have registered with our system.  If so, you will get a message stating that information already exists, and you should use the Existing User form to Log In.  If you do not remember your user Name and/or Password, there are links on the Home page that will assist you in recovering this information.

User Name

Your User Name should be something you can remember easily.  It cannot contain any spaces or special characters.
You will use this User Name to log into the My-PHA site to access your application and contact information.

Password
Verify Password

This can be anything you like, however, as in the case of your User Name, it should be something you can remember. 
Case is not considered, so it can be upper or lower case without a problem. 
Many sites recommend that you use numbers and letters to make the password more secure. 
We ask for you to type it twice to ensure that what we set for your User Name is exactly what you intend.

First and Last Names

This is your contact information, and we ask for it in case we need to contact you for any questions concerning your application.

E-Mail Address

This field is not required, but it can be helpful in contacting you should anything need to be discussed concerning your application.

Street, City, State, Zip, and Phone

This is your contact information, and we ask for it in case we need to contact you for any questions concerning your application.

If your registration is successful, you will be taken to the My PHA Online Options page.
When you return to MyPHA in the future, you will need only to log in using the Existing Account  form on the left of the Log In page.
